Leave Applications

    Unpaid and maternity/parental leaves should be applied for using the leave application found in PAWS.

    1. Login to PAWS and access the Leave of Absence Application.

    2. Choose the "Type of Leave" from the drop down menu.

    • Please note: faculty should only use this form to apply for personal or maternity/adoption/parental leaves. For all other leaves, refer to your collective agreement for further information.

    1. Enter your “First day off salary”.

    • If your first day off work is a Friday, your first day off salary is your next scheduled work date, usually Monday.

    • In the case of Maternity/Adoption/Parental leaves, enter the first day of your leave regardless if it is paid or unpaid. The cost of benefits calculated on the form is for the unpaid portion of your leave.

    1. Select your “Last day off salary” (This is the day before you return to work).

    2. Select “Benefit payment".

    • Select how you would like to pay for any benefits for which you selected "Yes" in the chart below. Please note that the values which appear are approximate and should be used to assist you in making a decision regarding benefit maintenance while on leave. These costs are subject to rate change. As such, you should not submit post-dated cheques based on these amounts.

    • If you select “Deduction from Final Cheque” the total premium amount for your benefits will be deducted from your last cheque prior to the commencement of your unpaid leave.

    • If you select “Submit Post-dated Cheques”, the Benefits Office will bill you, prior to the commencement of your unpaid leave, for the actual costs of your benefits to be paid on a monthly basis.

    1. Make your “Benefit decisions” for the unpaid portion of your leave.
    • Select “Yes” to continue the benefit(s). The total cost to maintain the benefit during the unpaid portion of the leave is displayed.

    • Select “No” if you choose not to continue the benefit(s). The cost of the benefit will disappear and the “Total Benefit Cost Estimate” will be recalculated.

    • Please note the values that appear are approximate and should only be used to assist you in making a decision regarding benefit maintenance while on your unpaid leave. DO NOT submit post-dated cheques based on these amounts, as these costs are subject to rate change.  

    • If any portion of your leave is paid, benefits will continue as if you were actively at work.

    1. Select “Download Form” to complete the application (this may take a few seconds).
    • Review, print and sign your form.

    • Submit it to your department/college for approval and processing.

    Partial Leave Without Pay
    • If your new FTE is greater than or equal to .5 you have the option of maintaining your disability, basic group life insurance, and pension at your full time salary, if you pay both employee and employer costs

    • Extended health care and dental benefits are not affected by a partial leave unless you drop below .5 FTE

    • If you choose to continue any of your benefits, you must continue them for the full period of your partial leave of absence

    • To calculate your new FTE divide the number of hours that you work per week by the total full-time hours per week.  Enter a number between 0 and 1. (Example: If full-time hours are 40 hours per week and you are working 32 hours per week, then your FTE calculates as .8 (30 hrs divided by  40 hrs)).

    • If your FTE drops below .5 you will be required to pay the full cost (employee and employer) for any benefits you choose to continue

    • Benefit costs for partial leaves will be deducted from your monthly pay

    • Basic group life and disability insurance can only be topped up for a maximum of 3 years


    Additional Leave Information
    • Housing Loan payments must continue to be made via post-dated cheques.

    • Computer loans from the Campus Computer Store must be paid in full. Payroll will collect the balance owing from your final pay before you go on leave. If the entire balance cannot be recovered from your final cheque, the computer store will contact you directly.  If you have questions, contact the Campus Computer Store at (306) 966-8375.
